The Nine-Year Gap

Life expectancy at birth is one of the most powerful summary statistics in public health. It captures infant mortality, access to healthcare, diet, violence, pollution, poverty, and dozens of other factors in a single number. And in the United States, that number varies enormously by state.

An 8.8-year gap between two American states is not a small rounding error — it is the difference between a generation that sees its grandchildren graduate high school and one that does not. It is the same gap that separates the United States as a whole from countries like Costa Rica and Chile. The question is not whether the gap exists. The question is why.

Epidemiology vs. medicine: Medicine asks "how do we treat this patient?" Epidemiology asks "why are people in this zip code, state, or country getting sick at this rate?" This module practices epidemiological thinking — looking for patterns across populations rather than treatments for individuals.

Notice the geographic pattern: the lowest life expectancy states form a cluster in the Deep South and Appalachia. The highest cluster in the Northeast, Pacific Coast, and Mountain West. The Social Determinants of Health

Public health researchers use the term social determinants of health to describe the non-medical factors that drive health outcomes: income, education, housing quality, food security, environmental conditions, and access to care. Research consistently shows these factors explain more of the variation in life expectancy than healthcare access alone.

📌 Mississippi vs. Hawaii — the whole story in two states: Mississippi has the lowest median household income in the U.S. (~$52,000), the highest poverty rate (~19%), the highest obesity rate (~40%), and the lowest life expectancy (71.9 years). Hawaii has the highest median household income (~$85,000), one of the lowest poverty rates (~9%), and the highest life expectancy (80.7 years). The same variables that predict political outcomes also predict how long people live.

Race, Place, and Unequal Outcomes

The state-level life expectancy gap is partly explained by income — but income does not explain everything. Black Americans have historically had lower life expectancy than white Americans at every income level, a gap driven by chronic stress from discrimination, differential access to quality healthcare, and residential segregation that concentrates pollution and poverty.

The COVID-19 pandemic made this visible in real time. Black, Latino, and Indigenous Americans died at significantly higher rates than white Americans — not because of biological difference, but because of pre-existing disparities in income, housing density, essential worker status, and access to care that the pandemic exposed and amplified.

The analytical question: When you find that states with higher Black population shares tend to have lower life expectancies, you have identified a correlation. The public health work is explaining why — distinguishing the effect of poverty, the effect of healthcare access, and the effect of racism as an independent stressor. Correlation is where the inquiry begins, not where it ends.

  1. Find your state in the life expectancy rankings and the income rankings. Are they in the same tier? If your state ranks high on income but low on life expectancy (or vice versa), what factors might explain that mismatch?
  2. The U.S. spends more per capita on healthcare than any other wealthy nation, yet ranks below most of them in life expectancy. Using what you know about social determinants, propose one explanation for why spending more on healthcare does not automatically produce better population health outcomes.
  3. An 8.8-year gap in life expectancy between the highest and lowest states has existed for decades. Is this a public health problem, a political problem, an economic problem — or all three? What would need to change to close it?
  4. The COVID-19 pandemic disproportionately killed Black and Latino Americans. Using the concepts from this module, identify three specific social determinants that contributed to this disparity — and explain the mechanism for each one.
